It is mandatory to have one completed Guest Info Form for each traveler.
PLEASE BE SURE YOUR NAME IS EXACTLY AS APPEARS ON YOUR PASSPORT (and check your Passport Expiration Date! It must be valid for 6 months after travel.
I have limited airline seats in group airfare & rooms on hold. Once those are filled, new requests to join the group will be on a request basis at the prevailing rates.
Space will not be confirmed until a completed form and deposit is received.
If you are in the process of getting or renewing your passport, just put 000 in passport section and advise when you've received your new passport.
